Three reasons levels fall at once
The first reason is intake. Alcohol supplies energy without meaningful nutrition, and heavy drinking displaces meals. A person can be taking in plenty of calories and very little of what the body needs to run.
The second is absorption. Alcohol irritates the lining of the stomach and small intestine and interferes with the transport systems that move thiamine, folate and B12 across the gut wall. Damage to the pancreas and liver compounds this, because both are central to digesting and storing nutrients.
The third is loss. Alcohol acts as a diuretic and increases urinary excretion of magnesium and zinc in particular. Vomiting, diarrhoea and sweating during withdrawal add to that.
Those three mechanisms stack, which is why deficiencies can appear quickly and why food alone sometimes needs medical support to correct.
Thiamine, the one that matters most
Thiamine is vitamin B1, and the brain depends on it to metabolise glucose. Stores are small, roughly two to three weeks worth, so they run down fast when intake stops and absorption is impaired.
Severe deficiency causes Wernicke encephalopathy, a medical emergency whose signs include confusion, unsteady walking and eye movement problems. Left untreated it can progress to Korsakoff syndrome, which involves permanent memory damage. This is the reason emergency departments and withdrawal services give thiamine routinely, often by injection at first, because oral absorption may be unreliable in the people who need it most.
This is the part of the article where self-management is the wrong approach. If someone is withdrawing from alcohol, or has been drinking heavily for years, thiamine should be handled by a physician. Food sources for maintenance afterwards include pork, sunflower seeds, legumes, whole grains and fortified breakfast cereals.
Magnesium
Magnesium is lost through the urine at higher rates with alcohol use, and low levels are common in heavy drinkers. It is involved in muscle and nerve function, and low levels can show as muscle cramps, twitching, irritability, poor sleep and palpitations. Those overlap heavily with the general discomfort of early recovery, which makes it easy to overlook.
Food sources are unremarkable and effective: pumpkin seeds, almonds, cashews, spinach and other dark leafy greens, black beans, whole grains and dark chocolate.
Magnesium supplements are widely sold and generally well tolerated, but they interact with some medications and are not appropriate at higher doses in kidney disease, so this remains a physician conversation.
The other B vitamins, zinc and vitamin D
Each of these is worth knowing about individually, because the symptoms are specific enough to be recognisable.
- Folate (B9). Low levels are common and contribute to anaemia and fatigue. Found in leafy greens, legumes, asparagus, avocado and fortified grains.
- B12. Absorption depends on a healthy stomach and terminal ileum, both of which alcohol affects. Deficiency causes fatigue, pins and needles and cognitive fog. Found in meat, fish, eggs, dairy and fortified foods.
- B6. Needed for neurotransmitter production, so low levels can affect mood and sleep. Found in poultry, fish, potatoes, chickpeas and bananas.
- Zinc. Losses rise with alcohol use, and low zinc affects immune function, wound healing, appetite and taste. Found in oysters, beef, pumpkin seeds, lentils and cheese.
- Vitamin D. Frequently low in this population and low across coastal British Columbia in winter generally. It affects bone health, mood and immune function. Found in oily fish and fortified milk, and made in skin exposed to summer sunlight.
How long repair takes
With regular eating, water-soluble vitamins such as folate and B6 begin to normalise within weeks. Magnesium and zinc follow over a similar period once losses stop. Vitamin D takes longer, and in a northern winter it often needs supplementation, which is precisely why that decision belongs with a physician who can test levels.
Liver recovery runs on its own timeline and depends on how much damage occurred. Fatty liver changes can improve substantially within weeks of stopping. Fibrosis and cirrhosis are different, and require ongoing medical follow-up.
Practical eating that covers most of this
Nobody needs to eat by spreadsheet. A pattern that covers nearly all of the above is straightforward and inexpensive.
- A protein food at every meal, rotating between eggs, fish, meat, dairy and legumes across the week.
- Oily fish twice a week for vitamin D and omega-3 fats.
- A daily handful of seeds or nuts, especially pumpkin seeds and almonds, for magnesium and zinc.
- Dark leafy greens most days for folate and magnesium.
- Whole grains rather than only refined ones, for B vitamins and steadier energy.
- Regular meal times, because absorption improves when eating is not chaotic.

- Which vitamin does alcohol deplete the most dangerously?
- Thiamine, or vitamin B1. Stores last only two to three weeks and severe deficiency can cause Wernicke encephalopathy, which is a medical emergency. This is why thiamine is routinely prescribed during and after alcohol withdrawal.
- Should I buy a high-strength B complex?
- Speak with a physician first. Doses matter, some B vitamins interact with medications, and high-dose B6 over long periods can cause nerve problems. Testing beats guessing.
